CDC+ Consumer Directed Care Plus
CDC+ is a long-term care program alternative to the Medicaid Home and Community-Based Services (HCBS) Medicaid Waiver.

The program provides the opportunity for individuals to improve the quality of their lives by being empowered to make choices about the supports and services that will meet their long-term care needs and to help them reach their goals.
Key Features of CDC+
Consumer-Directed Care Plus (CDC+) places participants in the driver’s seat. Individuals have more choice, flexibility, and control over their care. CDC+ consumers become employers with the ability to choose what services and supports they need and who will provide them.
Participant Empowerment:
Individuals are empowered to make decisions pertaining to their supports and have the flexibility to determine, choose, and pay for services through a monthly budget.
Choice of Service Provider:
The program provides individuals with unique benefits such as the ability to select providers who are not on the Medicaid network and the ability to customize services to meet individual care goals.
Supports:
A trained CDC+ consultant is always available to provide guidance and assistance.
Management Options:
Participants can choose to manage the program on their own or appoint a representative to assist in managing their budget and help make decisions regarding their care.
